局部振动对脑卒中偏瘫患者躯体运动功能影响的Meta分析

目的:分析局部振动对脑卒中偏瘫患者躯体运动功能的影响。方法:从中国知网、万方、维普、中国生物医学数据库、Pubmed、Cochrane Library、Embase及Scopus数据库检索脑卒中偏瘫患者接受局部振动训练的相关文献,检索文献时间段为2013年1月-2022年12月,经过筛选文献、提取数据后运用Revman 5.4进行数据分析。结果:本研究共纳入13篇文章,共526人。Meta分析结果显示,与常规康复训练相比,局部振动可改善脑卒中偏瘫患者的运动功能(SMD=1.04,95%CI:0.70, 1.37)、自理能力(SMD=0.96,95%CI:0.23, 1.69)及步行能力(SMD=1.27,95%CI:0.35, 2.19)。亚组分析结果显示,干预时间≤4周或>4周,局部振动均能改善脑卒中偏瘫患者的运动功能,而且在>4周亚组中,该效应更大(SMD=1.25,95%CI:0.84, 1.66)。结论:局部振动能有效改善脑卒中偏瘫患者的躯体运动功能,且治疗效果随干预时间的延长而提高。

Abstract:

Objective: To analyze the effect of local vibration on motor function in stroke patients with hemiplegia.Methods: Relevant literature of local vibration training on stroke hemiplegic patients was retrieved from CNKI, Wanfang, VIP, China Biomedical Database, Pubmed, Cochrane Library, Embase and Scopus databases from January 2013 to December 2022. After literature screening and data extraction, Revman 5.4 was used for data analysis. Results: There were 13 studies with a total of 526 participants were included in this study. The results of meta-analysis showed that local vibration could improve the motor function of stroke patients with hemiplegia(SMD=1.04, 95%CI: 0.70, 1.37), self-care ability(SMD=0.96, 95%CI: 0.23, 1.69) and walking ability(SMD=1.27,95%CI: 0.35, 2.19), when compared to conventional rehabilitation training. The results of subgroup analysis showed that local vibration improved motor function in stroke patients with hemiplegia both in intervention duration ≤4 weeks group and >4 weeks group, and with greater effect in the latter(SMD=1.25, 95%CI: 0.84, 1.66).Conclusion: Local vibration can effectively improve the physical motor function of stroke patients with hemiplegia, and the therapeutic effect increases with the extension of intervention time.
