目的:测试中文版Morse跌倒评估量表在ICU住院患者中的信度和效度,以期为ICU患者的跌倒评估测量和分析提供依据。方法:采用回顾性病例资料提取的方法,以2017年1月～2017年9月在我院ICU住院患者为研究对象,对ICU住院患者进行Morse跌倒评估量表评分。采用克朗巴赫系数(Cronbach's)α、内容效度、结构效度和因子分析方法对量表进行信度和效度的检验。结果:在ICU住院患者中,中文版Morse跌倒评估量表的内部一致性信度Cronbach'sα=0.067,与该量表信度Cronbach'sα=0.97的结果相比较,差距较大。内容效度评价:由5位N3级护士对量表的内容效度进行评价,结果显示:量表I-CVI≤0.80,量表S-CVI/UA=0.07,S-CVI/Ave=0.08,随机一致性概率(PC)值<0.031,K*(kappa值)<0.7。该量表的所有条目评价为较差。结构效度评价:因子分析结果显示:KMO值为0.472,Bartlett球形检验的统计量值40.08,P<0.01,KMO检验和Bartlett球形检验结果表明,该量表资料适合做因子分析。提取公因子:根据特征值>1来确定公共因子个数。研究结果显示:在特征值>1,初步提取3个公因子,正交旋转之后的累计方差贡献率分别为:48.27%,81.51%,91.92%。因子正交旋转之后量表的主成分1中X6>0.05,主成分2中的X1>0.05。结论:本研究结果显示中文版Morse跌倒评估量表在ICU住院患者中的信度和效度较低,未达到原量表的信度和效度的标准,在ICU住院患者中的适用性有待进一步研究和探讨。
Abstract：Objective:The purpose of this study is to test the feasibility,internal consistency reliability and validity of the Chinese version of the Morse Fall Assessment Scale for ICU hospitalized patients,so as to provide a basis for assessing the measurement and analysis to the patients with ICU falls.Methods:The retrospective case method was used to extract data from hospitalized patients in our hospital ICU between January2017 and September 2017.Nurses took the Morse Fall Assessment Scale to test ICU hospitalized patients so as to complete the score of the scale.The reliability and validity of the scale were tested by Cronbach'sα,content validity,structural validity and factor analysis.Results:In the ICU hospitalized patients,the Chinese version of the Morse Fall Assessment Scale was Cronbach'sα=0.067,which had a hug gap with the results of the English version of the scale of the Cronbach's α=0.97.Content validity assessment was conducted by 5 nurses of N3 level.The results were displayed as scale I-CVI≤0.80,S-CVI/UA=0.07,S-CVI/Ave=0.08,random consistency probability(PC)value<0.031 and K *(kappa value)<0.7,which were not considerable.The results of factor analysis showed that the KMO value was 0.472.The Bartlett spherical test was40.08 and P<0.01.The results of KMO test and Bartlett sphere test showed that the data were suitable for factor analysis.Extracting common factors:the number of common factors were determined according to the eigenvalue>1.The results showed that the cumulative variance contribution rate after orthogonal rotation was 48.27%,81.51% and 91.92% respectively based on the eigenvalue>1.After the orthogonal rotation of the scale,the main component 1 of the scale was X6>0.05,and the principal component 2 was X1> 0.05.Conclusion:The Chinese version of the Morse Fall Assessment Scale in ICU hospitalized patients has less reliability and validity,which is far from the standard of original scale.
