高强线联合带袢钛板固定在前交叉韧带胫骨止点骨折中的治疗效果

目的:探讨关节镜下高强线联合带袢钛板固定在前交叉韧带胫骨止点骨折中的治疗效果。方法:选择2019年2月—2023年2月就诊于南通市通州区人民医院的50例前交叉韧带胫骨止点骨折患者,根据关节镜下治疗方式不同分为两组,关节镜下行高强线联合带袢钛板固定的患者为观察组(n=25),关节镜下行空心螺钉固定的患者为对照组(n=25)。观察两组患者临床疗效。结果:与对照组相比,观察组的手术时间明显延长[(56.94±5.28) min vs (48.35±5.11) min]、出血量[(9.43±1.67) mL vs (12.12±2.34) mL]明显减少、骨折愈合时间[(10.65±1.01)周vs(12.36±1.16)周]及负重开始时间[(5.13±0.32)周vs (6.21±0.45)周]显著缩短(均P<0.05);观察组术后1年的膝关节活动度[(121.32±10.55)°vs (99.74±8.48)°]、Lysholm评分[(90.51±6.85)分vs(80.58±6.74)分]、IKDC评分[(90.58±6.23)分vs(74.24±5.67)分]、Tegner评分[(8.35±0.64)分vs (7.32±0.57)分]均较对照组明显升高(均P<0.05)。结论:关节镜下高强线联合带袢钛板固定在前交叉韧带胫骨止点骨折中的治疗效果明显,能减少术中出血、促进骨折愈合、增加膝关节活动度并改善膝关节功能。

Abstract:

Objective: To investigate the therapeutic effect of arthroscopic high tenacity sewing thread combined with looped titanium plate fixation in the treatment of anterior cruciate ligament tibial insertion fracture. Methods: A total of 50 patients with anterior cruciate ligament tibial insertion fracture who were treated at Tongzhou District People's Hospital of Nantong City from February 2019 to February 2023 were selected. According to the different methods of arthroscopic treatment, they were divided into two groups: the observation group(n=25) were treated with arthroscopic high-strength wire combined with looped titanium plate fixation and the control group(n=25) were treated with arthroscopic hollow screw fixation. The clinical efficacy of the two groups was observed. Results: Compared with the control group, the observation group had significantly longer operation time [(56.94±5.28) min vs(48.35±5.11) min], significantly less blood loss [(9.43±1.67) mL vs(12.12±2.34) mL], significantly shorter fracture healing time [(10.65±1.01) weeks vs(12.36±1.16) weeks] and weight-bearing time [(5.13±0.32) weeks vs(6.21±0.45) weeks](all P<0.05). The knee range of motion [(121.32±10.55)° vs(99.74±8.48)°], Lysholm score(90.51±6.85) vs(80.58±6.74), IKDC score(90.58±6.23) vs(74.24±5.67), and Tegner score(8.35±0.64) vs(7.32±0.57) in the observation group were all significantly higher than those in the control group(all P<0.05). Conclusion: High tenacity sewing thread combined with looped titanium plate fixation has a significant therapeutic effect on the treatment of anterior cruciate ligament tibial insertion fracture, which can reduce intraoperative blood loss, promote fracture healing, increase knee range of motion, and improve knee function.
