QTc间期和NT-proBNP在评估急性脑梗死患者短期预后中的价值

摘要:

目的:探讨QTc间期、血清N端B型钠尿肽前体(NT-proBNP)及其联合在急性缺血性脑梗死患者短期预后中的预测价值。方法:回顾性分析2019年1月—2024年9月在宜昌市中心人民医院神经内科住院确诊急性缺血性脑梗死患者123例,治疗3个月后根据改良Rankin量表(mRS)评分将急性脑梗死患者分为预后良好组(n=63例)(mRS评分<3分)和预后不良组(n=60例)(mRS评分≥3分),比较两组患者的一般资料、QTc间期和NT-proBNP。采用Pearson相关性分析及多因素Logistic回归来分析急性脑梗死患者短期预后不良的独立危险因素,并绘制受试者工作特征(ROC)曲线。结果:两组患者一般临床资料比较,预后不良组年龄、QTc间期和NT-proBNP均高于预后良好组(均P<0.05)。Pearson相关性分析显示,患者年龄、QTc间期和NT-proBNP均与mRS评分呈正相关(均P<0.05)。多因素Logistic回归分析结果显示,年龄偏大(OR=1.056,95%CI:1.001,1.115)、QTc间期延长(OR=1.018,95%CI:1.003,1.035)及NT-proBNP升高(OR=1.010,95%CI:1.004,1.017)是急性脑梗死患者短期预后不良的独立危险因素。年龄、QTc间期、NT-proBNP及三者联合预测急性脑梗死患者短期预后不良的ROC曲线下面积分别是0.825、0.804、0.752及0.864。结论:年龄偏大、QTc间期延长及NT-proBNP升高是急性脑梗死患者短期预后不良的独立危险因素,三者联合预测价值更高。

Abstract:

Objective: To explote the predictive value of the QTc interval, serum N-terminal pro-B-type natriuretic peptide(NT-proBNP), and their combination for short-term prognosis in patients with acute cerebral infarction. Methods: A retrospective analysis was conducted on 123 patients diagnosed with acute cerebral infarction who were hospitalized in the department of neurology at Yichang Central People's Hospital from January 2019 to September 2024. Based on the modified rankin scale(mRS) score, patients were classified into a good prognosis group(n=63)(mRS score < 3) and a poor prognosis group(n=60)(mRS score ≥ 3) after 3 months of treatment. Demographic data, QTc interval, and NT-proBNP levels were compared between the two groups. Pearson correlation analysis and multivariate Logistic regression were performed to identify independent risk factors associated with poor short-term prognosis, and the receiver operating characteristic curve was drawn. Results: Compared with the good prognosis group, the poor prognosis group had significantly higher levels of age, QTc interval, and NT-proBNP(all P<0.05). Pearson correlation analysis revealed that age, QTc interval, and NT-proBNP were positively correlated with mRS scores(all P<0.05). Multivariate Logistic regression analysis demonstrated that advanced age(OR=1.056, 95%CI: 1.001, 1.115), prolonged QTc interval(OR=1.018, 95%CI: 1.003, 1.035), and elevated NT-proBNP(OR=1.010, 95%CI: 1.004, 1.017) were independent risk factors for poor short-term prognosis in patients with acute cerebral infarction. The areas under the ROC curve for age, QTc interval, NT-proBNP, and their combined model in predicting poor prognosis were 0.825, 0.804, 0.752 and 0.864, respectively. Conclusion: Advanced age, prolonged QTc interval, and elevated NT-proBNP are independent risk factors for poor short-term prognosis in patients with acute cerebral infarction. The combined assessment of these factors provides greater predictive accuracy.
