目的:探讨CT灌注成像参数与乳腺非特殊型浸润性癌组织中Ki-67阳性表达率的相关性。方法:选择本院经术后病理证实的35例乳腺非特殊型浸润性癌患者为研究对象,术前采用西门子公司的炫速双源CT对所有患者进行胸部动态容积灌注扫描,经双源CT工作站后处理测量肿块实性部分的血流量(BF)、血容量(BV)和达峰时间(TTP),采用免疫组化检测乳腺癌组织中Ki-67的阳性表达率,分析各CT灌注参数与Ki-67阳性表达率之间的关系。结果:肿瘤大小与Ki-67阳性表达率呈正相关(P=0.001,r=0.523),BF与Ki-67阳性表达率呈正相关(P=0.001,r=0.526),TTP与Ki-67阳性表达率呈负相关(P=0.004,r=-0.470),而BV与Ki-67阳性表达率无相关性(P=0.156)。结论:双源CT低剂量灌注成像参数可以评估乳腺非特殊型浸润性癌Ki-67阳性表达率,为术前了解肿瘤细胞的增殖活性提供一定的参考价值。
Abstract:Objective:To investigate the correlation between the positive expression rate of Ki-67 and CT perfusion imaging parameters in non-specific invasive breast carcinoma.Methods:A total of 35 patients with non-specific invasive breast cancer confirmed by postoperative pathology in our hospital were scanned with dynamic volume perfusion of chest with Siemens' hypersensitive dual-source CT before operation.The blood flow(BF),blood volume(BV)and time to peak(TTP)of the solid part of the mass were measured by the post-processing of the dual-source CT workstation.The positive expression rate of Ki-67 in breast cancer tissue was determined by immunohistochemistry.The relationship between CT perfusion parameters and positive expression rate of Ki-67 were analyzed.Results:Both the tumor size and BF were positive correlated with Ki-67 expression rate(P=0.001,r=0.523;P=0.001,r=0.526);and TTP was negative correlated with Ki-67 expression rate(P=0.004,r=-0.470),but no correlation between BV and Ki-67 expression rate(P=0.156).Conclusion:Imaging parameters of dual-source CT low dose perfusion could be used to evaluate the positive rate of Ki-67 expression in non-special invasive breast cancer,which would provide reference to understand the proliferative activity of tumor cells before operation.
