目的:探讨间歇性气压泵治疗联合脚踏车训练预防ICU卧床患者下肢深静脉血栓(LEDVT)形成的临床应用效果。方法:选取2016年3月~2018年3月我院ICU住院患者118例,按照随机数字表法将患者随机分为对照组和观察组。对照组61例,观察组57例,对照组采用间歇性气压泵治疗预防LEDVT,观察组在对照组基础上增加脚踏车训练。后由同一超声科医师在超声下检测两组患者下肢深静脉平均血流速度变化及LEDVT发生情况。结果:观察组患者LEDVT的发生率为3.51%,显著低于对照组的18.03%,差异有统计学意义(P<0.05)。两组患者治疗前双下肢深静脉平均血流速度差异无统计学意义(均P>0.05)。与对照组比较,观察组治疗后4d的双下肢深静脉平均血流速度均明显增快(均P<0.05),且观察组治疗后7d的股深静脉、股浅静脉和腘静脉平均血流速度均明显增快(均P<0.05),双侧股总静脉、胫后静脉平均血流速度有增快趋势,但差异无统计学意义(均P>0.05)。结论:间歇性气压泵治疗联合脚踏车训练可提高下肢深静脉平均血流速度,有效降低LEDVT的发生率。
Abstract:Objective:To explore the clinical effect of intermittent pneumatic pump combined with bicycle training on preventation of lower extremity deep vein thrombosis(LEDVT)in ICU patients.Methods:A total of 118 inpatients in department of intensive care unit from March 2016 to March 2018 were selected.Then the patients were randomly divided into the control group and the observation group using random number table method.The cases in the control group(n=61 cases)were given intermittent pneumatic pump.The cases in the observation group(n=57 cases)was added with bicycle training on basis of the control group.The mean blood flow velocity of lower extremity deep vein and the incidence of LEDVT were measured by the same ultrasound doctor.Results:The incidence of LEDVT(3.51%)was obviously lower in observation group than that of control group(18.03%),and the difference was statistically significant(P<0.05).There was no statistically significant difference in the mean velocity of blood flow in lower extremity deep veins between the two groups before treatment(All P>0.05).Compared with the control group,the mean velocity of blood flow in lower extremity deep veins were increased obviously at 4 days after treatment in the observation group(All P<0.05).Moreover,the mean velocity of blood flow in the deep femoral vein,the superficial femoral vein and the popliteal vein were all extremely enhanced at 7 days after treatment in the observation group when compared to the control group(All P<0.05).However,the mean velocity of blood flow in bilateral common femoral vein and posterior tibial vein increased rapidly at 7 days after treatment in the observation group without statistical significance(All P>0.05).Conclusion:Intermittent pneumatic pump combined with bicycle training can effectively reduce the incidence of LEDVT and improve the mean velocity of blood flow in lower extremity deep veins.
